Last week, another child died in youth detention in Western Australia, increasing pressure on the Federal Government to find long-term solutions that make the system less harmful to children. 

National Children's Commissioner Anne Hollonds told RN Breakfast a Commonwealth coordinated approach to child safety is needed and warns there's no evidence that "tough on crime" policies lead to better outcomes for children.
