Those are the stark words of the midwife Donna Ockenden - the author of the long awaited Ockenden Review - published yesterday. Her mission? To find out what went on under the care of those working for the Shrewsbury and Telford Hospital NHS Trust over a 20 year period. She concluded 201 babies and nine mothers could have survived if the Trust had provided better care, learned from mistakes and crucially listened to women.
