When it comes to submission forms the more you ask for, the less you'll receive. Every additional question you toss in there is a chance your prospect will say "No Thanks" and leave before submitting their response.
Keep it simple and only request what's absolutely necessary. For instance, if you're not planning to give them a ring, why bother asking for their phone number, right? Stick to the basics – name and email address will do the trick. Start by sending them valuable info, and as you build that trust, you can gradually request more.
And don't forget the form's design – it's a big deal! Many folks will be filling it out on trusty mobile devices, so make it mobile-friendly by arranging the questions in a single column, aligned to the left.
