Star Trek has had a lot of holodeck stories where a malfunction has posed danger to the crew, but the concept rarely gets executed better than Voyager's third season episode, "Worst Case Scenario."

A tactical training program written by Tuvok becomes entertainment for the crew, and trying to write an ending executes potentially deadly subroutines left behind by Seska.
